As the Social Care Network Lead Entities continue to work with NYS on finalizing their contract details, some of the Lead Entities have begun accepting applications and sharing initial information about the implementation process. We want to pass along some of this information to help you engage in this potential opportunity to receive Medicaid funding for providing certain Health Related Social Needs (HRSN) services.
If your organization provides services or would like to start providing services such as the ones listed below, then you may be able to be reimbursed under the 1115 Waiver:
Health Related Social Needs Screenings
Referral Services / Navigation
Care Navigation / Case Management
Nutrition Supports such as:
Nutritionally appropriate food prescriptions (fruits, vegetables, and proteins)
Nutrition vouchers (redeemable at food retailers)
Pantry Stocking (fresh produce and healthy nonperishable groceries)
Other HRSN services such as housing and transportation
In the Capital Region, North Country Region, and Central New York Region, the first round of applications are due to your Lead Entity, HealthyAlliance, by September 30th, so we wanted to bring this to your attention (after September 30th, HealthyAlliance will accept applications on a rolling basis).
